After rebasing candidates onto the green tim tip, restore missing
EnvironmentThread loading fields, ChatView sendDisabledReason/threadSyncPhase
wiring, and orchestration.getThreadActivities auth coverage.

Send outbound comments with parentId under the mention (or thread root
when the user wrote inside an existing reply). Fall back to a top-level
comment if Jira rejects the parent.

* fix(server): recreate missing worktree on bootstrap

When a thread already has worktreePath/branch but the directory was
removed (disk GC or manual cleanup), re-run git worktree add at the
same path for the existing branch and re-run setup instead of assuming
the tree is still on disk.

* fix(server): use Effect FileSystem for worktree path check

node:fs/promises trips effect(nodeBuiltinImport) in the server typecheck
gate. Match the rest of the codebase with FileSystem.exists.

* test(server): fix bootstrap worktree reuse and cover recreate

The replay-reuse test used a non-existent worktree path; after exists()
checks that path no longer short-circuited prepareWorktree and hit an
unmocked fetchRemote. Create a real temp dir for reuse, and add a
missing-path recreate case.

Restore conversation-level AI usage status and host CPU/MEM/Load in ChatHeader,
add usage dots to Sidebar V2 thread rows (classic sidebar already had them),
and add anti-stack-drop existence tests so these surfaces cannot vanish on
product rejoin.

Identity is a registered integration overlay (compose already lists it)
but fork-ci pull_request.branches omitted it, so child PRs into
fork/identity never got Check/Test. Align workflow + docs with
pr-stack/compose.

ws.ts kept a parallel RPC_REQUIRED_SCOPE Map while RpcAuthorization.ts
owned the typed RPC_REQUIRED_SCOPES table. Identity methods were added
only to the typed table, so production failed with "has no declared
authorization scope" for identity.getSessionClaim.

Route observeRpc* through requiredScopeForRpcMethod and drop the Map.
